All documents are required, unless stated otherwise. Photos of documents are acceptable, as long as all the relevant information is clear. We'll only share these documents with the licensing authority.

What if I can't submit the documents digitally?

If you can't submit the documents digitally you can send copies by post instead. We'll give you the address and reference number to quote once you've paid the licence fee.

Photo of licensee
Passport type photograph of the proprietor/owner/licensee signed on the back.

Proof of ID
Proof of identification of licensee i.e. copy of driving licence, passport or utility bill dated within the last 3 months

Premises plan
Plan of the premises, including the treatment areas

Electrical certificates
Copy of fixed 5 year electrical wiring certificates for the building or electricians hand over certificate if premises are a new build

PATS (small electricals testing)
Copies of test certificates for portable electrical equipment used for the business or proof of newly purchased equipment.

Brochure
A current and up to date copy of your customer brochure that shows treatments offered and prices charged

Relevant training certificates or diplomas

I/We declare that the foregoing particulars are true in every respect. I/We hereby give notice to permit Thurrock Council to contact the Chief Officer of Police of my/our intention to apply for a licence under Part VI of the Essex Act 1987.
