Following on from this consultation, we have updated the certificate of vision impairment (CVI) form and the explanatory notes for consultant ophthalmologists to refer to when completing the form.

Consultation description
The CVI and the ‘Explanatory notes for consultant ophthalmologists and hospital eye clinic staff in England’ (ENs) have been revised with a group of stakeholders led by the Department of Health and the Royal College of Ophthalmologists to clarify who should be certified severely sight impaired and sight impaired.
The CVI form has been revised to update contact details and simplify terminology, and ensure it captures the information to support people who may need help from local authorities.
The explanatory notes have been updated to give clearer guidelines to who should be certified with sight impairment. The criteria for certification are based on visual acuity, extent of visual field loss together with the clinical judgement of the consultant ophthalmologist.
This consultation seeks views on the revised CVI form and explanatory notes and suggestions on how they can be improved further.
