Defence Accident Investigation Branch (DAIB)
The Defence Accident Investigation Branch (DAIB) provides defence with an accident and incident investigation capability independent of the single services and the defence safety regulators.
Who we are
The DAIB forms part of the Defence Safety Authority (DSA), with the Head of DAIB reporting directly to the Director General DSA (DG DSA).
Following the formation of the DSA on 1 April 2015, DG DSA became the primary Convening Authority for Service Inquiries pertaining to safety-related fatalities, serious injuries and major equipment failure or loss.
In October 2015 the DAIB formed, combining safety investigation staff drawn from the DSA regulators and the Front Line Commands. The creation of the DAIB assures a consistently high standard of independent accident investigation and reporting capability, as well as unified recommendation tracking and closure of all safety investigation recommendations. DAIB delivers a 24/7 on-call investigation capability able to deploy worldwide to investigate safety-related occurrences across all domains.
What we do
The Defence Accident Investigation Branch (DAIB) provides defence with an independent, multi-modal accident investigation capability. The DAIB conducts impartial and expert no-blame safety investigations, across all domains, focussed on the prompt identification and understanding of all accident factors. The investigation reports make pertinent safety recommendations which are tracked to closure by the DAIB.

Mission
To conduct independent safety investigation of accidents, serious incidents and near misses across defence, to determine causal factors and make targeted safety recommendations, in order to reduce the likelihood of reoccurrence and to preserve operational capability.
