1. 27 July 2023

   This 4-digit procedure to 3-digit additional procedure code correlation matrix layout and formatting has been updated. Technical content remains unchanged.

2. 17 November 2022

   Corrections have been made so the matrix fully reflects the acceptable combinations of Procedure and Additional Procedure codes.

3. 20 September 2022

   We have updated the correlation matrix to include 0020 and 21V.

4. 10 November 2021

   Additional Procedure Code 19Z is no longer available and has been removed from this guidance.
