Details

To apply for a Safe Manning Document (SMD) for a Merchant vessel, please complete this application form and email this to safemanning@mcga.gov.uk including a copy of the SMD held by any sister ships on the UK Ship Register (if applicable).

• for a large yacht SMD, please apply using form MSF 4208 instead
• for a fishing SMD, please apply using form MSF 3902 instead
• for a workboat/HSOSC SMD, please apply using form MSF 3914 instead
