Complete part C3 if you want to apply to vary (change) the conditions or any other part of your permit.

You will also need to complete part:

• A: about you
• C2: vary a bespoke permit
• F1: charges and declarations

If you are applying to vary or add a point source emission to water or sewer from an installation, you will also need to complete part C6 vary a water discharge activity, groundwater (point source) activity, or point source emission to water from an installation.

You will need to use an Adobe Acrobat Reader product to complete the forms.

Applications we receive from 7 March 2022 must be on this updated version of the part C3 form. We will return any that are not.
