Use this form to apply for a trailer identification number (a ‘ministry number’) to be allocated to new-build trailers (category O2, O3 and O4) before you apply for permission to supply the trailer for use on the road.

Send the form online

  1. Download and fill in the application form.

  2. Save the application form on your computer.

  3. Send the application form to the Driver and Vehicle Standards Agency (DVSA) using the service to apply for a vehicle test or certificate for a lorry or trailer.

Your application may be delayed if you do not fill in all relevant sections.
