Use this form to apply to the Driver and Vehicle Standards Agency (DVSA) to:
- apply to become an authorised examiner (AE) - a sole trader, partnership or limited company authorised to run an MOT test station
- report a change in your details or circumstances
- change your AE designated manager (the person in charge of MOT testing in your business)
- change your current MOT test station, including changing classes of vehicle it can test, or changing test equipment
You can also use it to apply to:
- set up another new MOT test station
- take on an existing MOT test station
How to apply

Download and save the application form on your computer before you start to fill it in.

Open the form from your computer, fill it in and save it again.

Depending on your business and what you’re applying to do, you might need to download, save and then fill in the extra declaration sheets. The guidance notes explain what to do.

Email the forms to motadministration@dvsa.gov.uk. You need to attach any documents to support your application. The form and guidance notes tell you what documents to include.

Post any declaration of unspent criminal convictions your limited company or partnership has (England, Scotland and Wales), and copies of any criminal record check certificates (Scotland only) to DVSA - you cannot send them by email.
You can also post the form and documents to DVSA.
