Apply to make, modify or supply an ear tag

Cattle, sheep and goat ear tag manufacturers and suppliers must get approval to make or modify ear tags, or supply an approved ear tag.

Use this form to apply to:

  • make a new ear tag
  • modify an existing ear tag
  • supply a tag from another manufacturer which has already been approved

You need to:

  1. Open the application form in a PDF reader, such as Acrobat Reader.
  2. Download and save the form to your computer.
  3. Fill in the form and save it again.
  4. Complete a separate form for each tag you want to make or modify.

What to send with your application

You must send:

  • at least 6 samples of each tag, printed with the Crown logo and a number in the correct format (such as UK 654321 123456)
  • an ear tag applicator for fitting the tags
  • instructions for fitting the tags, including images showing how the tag will be put in the animal’s ear
  • technical drawings of the ear tag (including measurements)
  • your proposed quality control procedures
  • your proposed marketing materials, including the name and type of tag
  • certificates showing each tag conforms to electronic identification (EID) technical standards (EID tags only)
  • an original version of the current tag if you’re applying to modify a tag

What happens next

The Livestock Unique Identification System (LUIS) support team will carry out an initial test of the ear tags.

They will also send the tags and documents to an Animal and Plant Health Agency (APHA) vet who will carry out welfare tests.
