Use this form to apply for one of these types of test at a non-Driver and Vehicle Standards Agency (DVSA) site:
- a model report for a vehicle that cannot be fully examined at a DVSA site
- a tilt test or step deflection test for buses or coaches where specialist equipment is needed
- a TIR test for lorries and trailers for quicker border crossings
