Leaseholders or landlords can use this form to ask for a determination of the reasonable costs associated with the process of leasehold enfranchisement or leasehold extension.

There are 4 types of applications that can be made with this form. You can use this form to make an application under either:
- section 33(1) of the Leasehold Reform, Housing and Urban Development Act 1993
- section 60(1) of the Leasehold Reform, Housing and Urban Development Act 1993
- section 91(2)(d) of the Leasehold Reform, Housing and Urban Development Act 1993
- section 21(1)(ba) and 21(2) of the Leasehold Reform Act 1967
