There are 4 types of applications that can be made with this form. You can use this form to make an application under either:

• section 33(1) of the Leasehold Reform, Housing and Urban Development Act 1993
• section 60(1) of the Leasehold Reform, Housing and Urban Development Act 1993
• section 91(2)(d) of the Leasehold Reform, Housing and Urban Development Act 1993
• section 21(1)(ba) and 21(2) of the Leasehold Reform Act 1967
