If you are deaf or have a hearing impairment and paid to take a British Sign Language (BSL) interpreter to your driving test, use this form to apply to the Driver and Vehicle Standards Agency (DVSA) for a refund.
Your claim will be delayed if you do not give all the information needed or send the right documents.
How to make a claim

Download and save the form to your device.

Open the form on your device and fill it in.

Print the filled in form and sign it.

Scan or take a clear photo of each page of the form.

Email the scan or photos to DVSA. You also need to attach scans or photos of a receipt if your interpreter did not sign the form.
