Use this form to apply to the Employment Appeal Tribunal to issue a penalty notice under regulation 33 of the European Public Limited-Liability Company Regulations 2004 or regulation 22 of the Information and Consultation of Employees Regulations 2004 or regulation 20, 21 or 21A of the 1999 Regulations.
