Completed forms should be sent to enquiries@groceriescode.gov.uk and served on the respondent in accordance with the CIArb Rules at the same time.

The form should:

  • set out the main elements of the dispute that will later form the basis of the statement of claim

  • summarise relevant events, give details of the alleged Code breach, and make reference to specific provisions of the Code and how they should be interpreted

  • give an indication of the loss suffered directly as a result of the alleged Code breach, together with details of any attempt to mitigate that loss, and the remedy sought
