Background
This memo informs you of amendments that have been made to the Access to Work MHSS Provider Guidance.

Chapter 4 paragraph 4.22 has been updated to remove reference to the consent form as it is no longer in use.

Chapter 7 paragraph 7.27, bullet point 3, sub-bullet point 6 has been removed as it refers to the consent form - no longer in use.

The latest version of Annex E already in use by providers has been published.
