Details

Healthcare providers should complete this form for any patient meeting the following case definition:

  • Mycobacterium avium complex, M. intracellulare or M. chimera endocarditis, surgical site infection or desseminated infection
  • AND who has had cardiopulmonary bypass (cardiothoracic surgery or extracorporeal membrane oxygenation or any other procedure involving the use of a heater cooler unit) in the 10 years before diagnosis
