Authorise the cremation of a dead person for deaths that occurred in Scotland, Northern Ireland or the British Islands

Form for medical referees to authorise the cremation of a deceased person's body. This form should be used for deaths that occurred in Scotland, Northern Ireland, the Channel Islands, or the Isle of Man, and where the cremation is to take place in England or Wales.

Details

This form must be completed by medical referees.

The form confirms that:

  • the cremation meets the regulations
  • the inquiry or examination made by the persons who gave the relevant certificates was adequate
  • the fact and cause of death have been agreed, or a coroner has opened an inquest

There is a different form if the death occurred in England or Wales and the cremation will take place in England or Wales.
