Details

Before a stillborn child can be cremated, a medical referee must authorise the cremation.

To do this, complete this Cremation 13 form which confirms that:

  • the cremation meets the regulations
  • the person who issued the certificate completed an adequate examination of the stillborn child
  • there’s no reason for further examination
