Form to complete a veterinary examination certificate for a wild bird which may be unfit to be released into the wild.

This form should be completed by a qualified vet to record the veterinary examination of a wild bird which the owner believes will not survive in the wild.
If the vet agrees that the bird cannot be released into the wild, the bird must be registered and microchipped.
