Complete this application form to apply to register a place of religious worship for the solemnisation of marriages under section 41 of the Marriage Act 1949.

Form 78A: certificate that consent has been given by the relevant governing authority (or authorities) for a building to be registered for the solemnisation of marriages of same sex couples. This form must be completed alongside form 78 for applications on or after 13 March 2014.
