Appendix A: application process for commercial providers

Review the information requested in the supplier checklist in Appendix B and submit all documentation that is relevant to your application.
The information you provide will be processed and made available to applicable personnel within the NHSCSP. By proceeding with the application, you are consenting to the collection and processing of your data in this way. Information you provide will be held by us and treated confidentially.
Please note that by submitting an application it does not guarantee your company will be asked to proceed with the process, nor does it imply that your company has any type of procurement relationship with the NHSCSP.
Your application must include:
- a company logo (on letterhead and email signature block)
- the name and address of company
- the name of a company contact
- the contact’s job title/company role
- a contact telephone number
- a contact email address
You should:
- provide website links to information that is available and published online
- cross reference all information to the relevant sections of the supplier checklist in Appendix B and submit this with your application
- save (or scan) all internal documents as PDF files
- attach documents in a zipped file(s), none of which should exceed 5 MB
Send your application to the chair of the Laboratory Technology Group by email at: PHE.screeninghelpdesk@nhs.net The subject line should state: FAO Chair – Laboratory Technology Group (Cervical Screening Programme).
Once your application is received, the chair or their representative will contact you at the earliest opportunity to advise if further information is required to process your application.
The LTG will review your application at the next scheduled meeting and make a recommendation on whether or not the test should be evaluated with a view to approving it for use in cervical screening.
The chair or their representative will contact you, normally within 10 working days, and advise on the next steps.
If your application is not recommended for evaluation, the chair or their representative will contact you, normally within 10 working days, and outline the reasons for this.
