Use this form while following the UK recovery handbook for chemical incidents (UKRHCI).

This form helps you plan and document how to manage and remediate contaminated food production systems, inhabited areas and water environments (including drinking water supplies) following a chemical incident.

To use the form, you need to enable macros (typically disabled for security reasons).
