Details

The terminal illness early access form should be completed by the person responsible for managing the account.

The child can claim if they’re:

  • over 16
  • managing the account

You’ll need to get the child’s medical practitioner to complete the early access medical report form if you are required to provide evidence that your child is terminally ill.

Send the early access medical report form (if applicable) with a copy of your terminal illness early access form to the address shown on the form.

Email HMRC to ask for this form in Welsh (Cymraeg).
