If you are applying to register with Ofsted because you are connected with a children’s social care establishment or agency, you will need to complete a ‘fit person’ or ‘suitable person’ questionnaire.
There are separate questionnaires depending on:
- whether you are joining a children’s home or another type of establishment or agency
- whether you are applying to be a registered manager or the responsible individual/registered provider
Children’s homes

Other children’s social care providers
The registered manager questionnaire helps Ofsted assess your fitness to be registered as a manager of:
- an adoption support agency
- an independent fostering agency
- a residential family centre
- a holiday scheme for disabled children
The registered provider or responsible individual questionnaire helps Ofsted assess your suitability to be registered as a provider of:
- a voluntary adoption agency
- an adoption support agency
- an independent fostering agency
- a residential family centre
- a holiday scheme for disabled children
