Complete this form if you want to become a member of a Facilitation Fund group. You can fill in the form electronically but you will need to print it to sign it.

Send the completed form to the group’s Facilitator so that they can include a copy of it with the group’s application for a Facilitation Fund agreement. Your Facilitator must keep the original signed version. You should keep a copy of the form for your records.
