British Sign Language (BSL) version of the COVID-19 vaccination consent form for children and young people or parents.
The consent form asks for information of a personal nature which we tell you about in this BSL video. Consent does not have to be in writing and therefore you may be asked to give your consent to vaccination either verbally or by non-verbal indication.
