Details

As the coronavirus (COVID-19) vaccination centres do not hold your medical records, vaccination checklists are designed to pick up any conditions that may need special consideration by a doctor or nurse before vaccination can go ahead safely.

Answering yes to one of these questions does not mean you or your child cannot have the vaccine but may mean the doctor or nurse needs more information to make sure you get the best protection. If you say no to all these questions, then the vaccination should be able to go ahead as planned.
