This document is important. Keep it safe. It confirms your COVID-19 status.
For each vaccine you have received: what the letter shows
- Name of vaccine and 2D barcode
- Dose: (for example: ‘Dose 4 of 4 (Booster)’)

- Vaccine manufacturer: (name of vaccine manufacturer)
- Disease targeted: COVID-19
- Vaccine product: (name of vaccine product)

The 2D barcode (the square patterns) on this pass expire after 180 days.

When and where could I be asked to show my NHS COVID Pass?
You may be asked to show your NHS COVID Pass if you travel abroad.
Is there a digital alternative?
You can access your NHS COVID Pass using the NHS App if you are 13 or over or the online NHS COVID Pass service. Find out who can use our digital services.
What should I do when the 2D barcode expires?
The square 2D barcodes on your pass have an expiry date. Your pass will not be accepted for international travel after that date.
Please destroy your pass securely when it expires and apply for a new one if you need it.
What should I do if I lose my NHS COVID Pass?
Your NHS COVID Pass is important. Keep it safe. It confirms that you have been vaccinated against COVID-19.
If you lose it, you can get information on what to do on the NHS website.
