Form for applying for the cremation of a deceased person. This form should be used for deaths that occurred in Scotland, Northern Ireland, the Channel Islands or the Isle of Man and where the cremation is to take place in England or Wales.

There is a different form if the death occurred in England or Wales and the cremation will take place in England or Wales.
