Some people are disqualified from providing, being involved in the management of, or being employed in, early years provision in schools.
Use this form to:
- describe any circumstances that you believe may disqualify you and Ofsted will confirm whether or not you are disqualified
- apply to Ofsted for consent to waive disqualification

This form has been updated following amendments to disqualification regulations under the Childcare Act 2006. Disqualification ‘by association’ no longer applies to childcare provision in schools.

We have updated the form to reflect current legal requirements about spent convictions under the Rehabilitation of Offenders Act, and to cover a new offence which prevents enforced subject access requests for criminal records.
