Employment intermediaries: report template
From 6 April 2015, employment intermediaries must use the template to send information about workers where they don't operate Pay As You Earn.

Details
The employment intermediary that holds the contract with the person who uses the worker’s services must send this information to HM Revenue and Customs. The intermediary must use the employment intermediaries service to upload and send their reports.
You can refer to the attached ‘How to use the employment intermediaries template’ or the help in the column and row headings of the template.
You can use the CSV and ODS versions of the template with all recent spreadsheet software like Excel, LibreOffice and OpenOffice.
If you use an Apple Mac, you’ll only be able to use the CSV version of the template with Numbers.
