How to complete the EU Attestation of Equivalence template

You must complete sections 1, 3, 6, 9 and 10 of the EU AoE template for all consignments.
You may also need to complete sections 5, 7 and 8 but this depends on if the consignment consists of either hops or hops products, and if the hops or hops products are made from blends of hops.
Section 4 of the EU AoE template is pre-populated.
We will also complete sections 2, 11 and 12.
For exports to the EU, the EU Customs Authorities will complete section 13 when your consignment arrives in the EU. For movements to NI, customs authorities in NI will complete Section 13 when your consignment arrives in NI.
The sections of the EU AoE template you need to complete are below.
Section 1: Consignor (full name and address)
Enter the name and full address of the GB person or business exporting the consignment of hops or hops products. This must be the address you previously enrolled with us.
Section 3: Consignee (full name and address)
Enter the name and full address of the person or business in the EU or NI you are exporting or moving the consignment of hops or hops products to.
Section 5: Place of production of hops
Hops
Enter the location of the place where the hops were grown, for example, Kent.
Hops product (not made from blends of hops)
Enter the location of the place where the hops used to make the product were grown, for example, Kent.
Hops product (made from blends of hops)
You do not need to complete this section but in Section 9 you must state:

the different varieties
-
the places of production of the hops
-
the percentage by weight of each variety from each production area making up the blend
Section 6: Harvesting Year
Enter the year the hops in this consignment were harvested, for example, 2018.
Section 7: Place of processing
Hops
You do not need to complete this section.

Enter the location of the place where the hops were processed, for example, Kent.

Enter the location of the place where the hops were processed, for example, Kent.
Section 8: Date of processing
Hops
You do not need to complete this section.

Enter the month and year the hops were processed, for example, October 2019.

Section 9: Marks, numbers, number and kind of packages – description of products – variety
Hops
Include the following.

Number of packages of hops or hops products included in your consignment

Description of each of the packages of hops or hops products (for example, weight, packaging)

Hops variety
The description of the products should be one of the following.
a. ‘Unprepared hops’ - hops which have only undergone preliminary drying and packaging
b. ‘Prepared hops’ - hops which have undergone final drying and packaging
The description must be followed by the word ‘Seedless’ if the seed content is less than 2% of the weight of the hops, and by the word ‘Seeded’ in other cases.

The description of the products should be one of the following:
c. ‘Hop powder’ (which includes hop pellets and enriched hop powder)
d. ‘Isomerised hop extract’ - an extract in which the alpha acids have been almost totally isomerised
e. ‘Extract of hops’ - other extracts than isomerised extract of hops
f. ‘Mixed hop products’ - a mixture of the products referred to in points (c), (d) and (e) above, excluding hops

Include the following:
