All registered community design applications should be accompanied with a supporting fee sheet.

Guidance notes on completing and submitting the form can be found on page 2 of this form.

Note: If you make payment by credit or debit card you will need to:

• make payment online
• make a note of the payment reference in the box provided on the fee sheet
• send the paper form(s) and fee sheet to the IPO
