Request for detailed information about hospital admissions for ‘acute reaction to foreign substance accidentally left during a procedure’ and ‘foreign body accidentally left in body cavity or operation wound following a procedure’ in 2014-15.

NHS Improvement held information relevant to the request but decided not to release it on the basis of the exemptions in section 12 (exceed the cost limit) and 21 (information accessible by other means) of the FOI Act.
