A request for details of ‘medical incidents’ that led to patient deaths at Brighton and Sussex University Hospitals NHS Trust, Western Sussex Hospitals NHS Trust and East Sussex Healthcare NHS Trust in 2015/16.

NHS Improvement held information on patient safety incidents where the reported degree of harm was death and decided to release it.
