Leaseholders or landlords can use this form to request a decision from the tribunal on price, terms of lease, or terms of acquisition associated with the process of leasehold enfranchisement or leasehold extension.

There are 6 types of applications you can make with this form.
You can make a collective enfranchisement of flats application under:
- section 24(1) of the Leasehold Reform, Housing and Urban Development Act 1993 – if the landlord is known
- section 27 of the Leasehold Reform, Housing and Urban Development Act 1993 – if the landlord is missing
You can make a lease renewal of flats application under:
- section 48(1) of the Leasehold Reform, Housing and Urban Development Act 1993 – if the landlord is known
- section 51 (1) to (5) of the Leasehold Reform, Housing and Urban Development Act 1993 – if the landlord is missing
You can make an enfranchisement of a house application under:
- section 21 of the Leasehold Reform Act 1967 – if the landlord is known
- section 21(1)(ba) and 21(2) of the Leasehold Reform Act 1967 – if the landlord is missing
