Freedom of Information on enquiry about Yellow Card report regarding infant medical overdose (FOI-20-579)

Thank you for your Freedom of Information (FOI) request dated 19th December 2020, where you requested the following information:

‘Has the MHRA been alerted to the medication overdose and circumstances and the death of the baby, either by the Chair of the investigation, anyone from the investigation team, the NMC or by Hampshire Constabulary who were also fully aware of this during their own investigation and by 2006 at the latest, or by any other norganisation or department?’

Upon review of the Yellow Card data that has been voluntarily provided to us by patients, carers, healthcare professionals and pharmaceutical companies alike, we have been unable to identify a reportnthat matches the descriptions posed within your initial email to us and therefore believe we are not in possession of this report.

To note, if you would like to this can be reported via the Yellow Card scheme, there is no time limit as to when incidents can be reported. Reporting is available via our online tool, here: https://yellowcard.mhra.gov.uk/. If you are dissatisfied with the handling of your request, you have the right to ask for an internal review.
