The above forms are the bilingual equivalents of the standard notice of marriage form and affidavit/affirmation form.
You can use them to make your affirmation with a local notary public in Russia in order to post your notice of marriage to British Embassy Moscow or Consulate General Ekaterinburg rather than apply in person.
