Defnyddiwch y ffurflenni hyn i wrthwynebu i rywun gofrestru atwrneiaeth arhosol (LPA) neu atwrneiaeth barhaus (EPA).

I wrthwynebu cofrestru LPA, defnyddiwch:

  • LPA006 os chi yw rhoddwr yr LPA ac rydych eisiau gwrthwynebu am unrhyw reswm
  • LPA007 os ydych yn atwrnai neu’n ‘unigolyn i’w hysbysu’ ac rydych eisiau gwrthwynebu ar sail ffeithiol
  • LPA008 os ydych yn unrhyw un (gan gynnwys atwrnai neu ‘unigolyn i’w hysbysu’) sydd eisiau gwrthwynebu am resymau eraill (sef ‘sail ragnodedig’)

Os ydych eisiau gwrthwynebu ar sail ragnodedig, mae’n rhaid ichi hefyd wneud cais i’r Llys Gwarchod a thalu ffi o £400.

I wrthwynebu cofrestru EPA:

I wneud cais am unrhyw ddogfen mewn fformat arall, megis Braille, recordiad sain neu brint bras, anfonwch e-bost i customerservices@publicguardian.gsi.gov.uk. Dylech gynnwys eich cyfeiriad a’ch rhif ffôn a theitl y ddogfen y mae arnoch ei hangen.
