The Health Services Safety Investigations Body (HSSIB) was formally established on 1 October 2023 as an independent investigations body with powers to conduct investigations into patient safety incidents in the NHS and independent sector.

As HSSIB is still within its first year of operation, this is its first annual report and accounts, covering the 6-month period 1 October 2023 to 31 March 2024.

The report:
