This form is part of the guide to burning animal by-products ABPs) at an incineration site.

Use this form to apply for approval to incinerate ABPs at your site, including animal carcasses or parts of carcasses.

The incineration of ABPs, or products derived from ABPs, must take place at an approved plant.

For customers in England, return the completed form to APHA Customer Service Centre Worcester.

For customers in Wales, return the completed form to the APHA office in Caernarfon.

For customers in Scotland, return the completed form to your local APHA Field Service Office.
