On 4 October 2021, Ofsted, HMIP and the CQC jointly visited Oakhill STC as part of their full annual inspection. As a result the inspectorates issued an Urgent Notification (UN) to the Secretary of State 14 October 2021 in relation to serious concerns about the safety and welfare of children. The former Prisons Minister Victoria Atkins commissioned a report into the safeguarding issues raised at Oakhill STC. Shelia Durr, a sector expert, was commissioned to undertake an independent review of safeguarding to provide significant expertise and objectivity.
The report made 19 recommendations to G4S, YCS and statutory safeguarding children partners, which the Government accepts, and is working with partners to implement.
