Part 3: About your work or your approved employment training scheme or course and your disease
Give as much detail as you can when you fill in ‘Part 3: About your work or your approved employment training scheme or course and your disease.’
We need at least 10 years employment information for your claim. You may want to think about the employers you worked for the longest. This could cover the 10 years needed and you may not need to give details of every employer.
What type of work or training do you think caused your disease?
When you answer the question ‘What type of work or training do you think caused your disease?’ please give us:
- employment details covering 10 years or more where you used hand-held vibrating tools
- for each employer how many hours a day and how many days a week you spent using hand-held vibrating tools
- the names of the tools used
- the jobs you performed
If you need to tell us about more than 2 employers
Use the last page of the claim form BI100PD to give us the extra information. Make sure you give the same information about these employers and the work you did as you give in Part 3.
Contacting your employers
We may need to contact any of the employers you tell us about.
