Mrs Lisa Ion in her capacity as Liquidator in 9 cases failed to take reasonable steps to ensure that the RP15 forms were submitted to the Redundancy Payments Service as soon as was reasonably practicable, as required by Section 125(3) of the Pension Schemes Act 1993. This was in breach of the fundamental principle of Professional Competence and Due Care as set out in R2103.1(b) of the Insolvency Code of Ethics (effective from 1 May 2020).

Mrs Lisa Ion is therefore liable to disciplinary action under Disciplinary Bye-law 4.1b (effective from 14 October 2019 to 31 May 2023 and effective from 1 June 2023).
