Your request has been dealt with under the Freedom of Information Act (FOIA).
I can confirm the agency holds the information that you have requested in question 1 and 2, and this information is available to you from:
- https://www.gov.uk/government/statistics/individual-insolvency-statistics-april-to-june-2022. The number of individuals entering an IVA in England & Wales is shown in table 1, Northern Ireland is shown in table 8, and the number entering Protected Trust Deeds in Scotland is shown in table 6. A Protected Trust Deed is the closest equivalent to an IVA in Scotland. The number of individuals entering bankruptcy is shown in the same tables.

Separate numbers for England and for Wales can be found in the Location, Age and Gender statistics.
The Insolvency Service is not obliged to provide you with a separate copy of information it holds where that information is reasonably accessible to the applicant.
As such, the information you have requested is exempt from disclosure under section 21 of the Freedom of Information Act 2000, and your request is refused.
The Insolvency Service does not hold the information you have requested in question 3.
Please note FOIA only applies to recorded information, it does not require public authorities to create information in order to answer a question.
