Use this form to declare that seat belt anchorages in passenger carrying vehicles meet safety standards. You have to do this when you get them approved under the Individual Vehicle Approval (IVA) scheme.
If a vehicle has extra seat belts fitted beyond those required by law, it must be tested if it’s either a:
- private passenger vehicle with more than 8 passenger seats, and was first used from 1 October 2001
- a public service vehicle (PSV)
How to send the form
Send the form along with your application to:
- apply for IVA for cars and passenger vehicles with no more than 8 seats
- apply for IVA for vans and light goods vehicles with a gross vehicle weight of up to 3,500kg
- apply for IVA for buses or coaches

Your application can be delayed if you do not fill in the declaration form correctly.
