Use this form if you have a long-term physical or mental condition and don’t need to:

A qualified medical professional should complete this form on behalf of applicants applying for settlement or naturalisation as a British citizen. If you already have a report or letter from a medical professional describing your condition, please include it with the form.

When submitting your completed Life in the UK exemption form, please include all original and current relevant medical reports setting out your diagnosis.

This exemption form should be included with the applicant’s visa application form.
