Use this form if you have a query about your lump sum forecast statement or post application statement.

Details
Farmers (or those acting on their behalf) should fill in this form if they have a query about the forecast statement or post application statement they have received.
Only fill it in after you have read your forecast statement or post application statement and checked the scheme rules.
Send your completed form to the Rural Payment Agency (RPA) by email or post.
If you’re sending more than one form by email, use a separate email for each form.
When to use this form
Use this form if you:
- think your Lump Sum Exit Scheme forecast statement or post application statement is wrong
- need to send RPA further evidence or information about a lump sum query you’ve already raised
- disagree with a recalculated forecast statement
Do not use this form to:
- query anything other than the forecast statement or post application statement
- chase a statement you haven’t received yet
- chase a lump sum query form you’ve already submitted

When you’ll hear back
RPA will send you an acknowledgement email or letter.
You’ll receive a response after your statement query has been reviewed. If we do not need to take any further action we’ll explain why. If we need to take further action, we’ll confirm what we’ve done and the effect this has had on the original statement by scheme year.
If you have another statement query that you have not already told RPA about, fill in and submit a new query form.
