Our current investigations table has been updated to list our latest deployment to the foundering of the UK registered, 23.1m fishing vessel Odyssey (FR70) approximately 130 nautical miles east of Fife Ness, Scotland on 29 October 2024. The crew were rescued from the vessel’s life raft uninjured.

The foundering of the Saint Helena registered vessel Argos Georgia in heavy seas approximately 200nm east of Port Stanley, Falkland Islands on 23 July 2024. A preliminary assessment also started last week into the grounding of the UK registered dive support vessel Jean Elaine in Deer Sound, Orkney on 22 July 2024.

Our current investigations table has been updated to list a preliminary assessment into the collision between Apache (9749489) and Serinah (GH116) resulting in the sinking of Serinah approximately 6nm southwest of Ardrossan in the Firth of Clyde, Scotland on 25 April 2024.

We have updated the entry for Viking Sky in the 'Investigations into accidents involving UK registered vessels and/or UK citizens that, by agreement, are being led by other administrations', to reflect the publication of the accident report by the Norwegian Safety Investigation Authority (NSIA). The table entry now contains a link to the NSIA webpage for the published report PDF and a short video summarising the accident and findings.

Our current investigations table has been updated to list our latest deployment, to the foundering of UK registered stern trawler Opportune (LK209) in the North Sea, approximately 36 miles east of Lerwick in Shetland on 24 March 2024. The crew were rescued from the vessel’s liferaft uninjured.

Three cases have been added to the current investigations table. The grounding and loss of fishing vessel Sustain at Loch Broom, Scotland in November and last week, the man overboard, presumed deceased, from fishing vessel Amadeus in the German Bight are under investigation. A preliminary assessment also started last week into the fatal man overboard from the 8.18m fishing vessel Nista off Luing, Scotland.

Our current investigations table has been updated following the loss of propulsion of the UK registered passenger vessel Spirit of Discovery in the Bay of Biscay on 4 November, resulting in multiple passenger injuries during heavy weather.

Following today's report publication, the RRS Sir David Attenborough case has been removed from our current investigations table. An ongoing investigation has also been added after a recreational diver was struck and fatally injured by Karin, a passing dive support vessel, in Scapa Flow, the Orkney Islands, Scotland.

The current investigation table has been updated following yesterday’s fatal collision between the Isle of Man registered general cargo vessel Verity and the Bahamas registered bulk carrier Polesie in the German Bight traffic separation scheme. The collision resulted in the sinking of Verity with 1 person declared deceased. Two of the remaining 6 crew members were recovered from the water and 4 are missing, with searches ongoing. This investigation is on behalf of the Isle of Man Ship Registry, in accordance with the MoU between the MAIB and Red Ensign Group Category 1 Registries of the Isle of Man, Cayman Islands, Bermuda and Gibraltar.

The status for the investigation into the fatal sinking of an unnamed migrant boat on 24 November 2021 has been updated. It is intended that the investigation report will publish on 9/11/23. The status for the fatal accident on board the RIB Seadogz has also been changed to consultation stage.

We started two investigations into accidents resulting in a fatality on UK registered vessels last week - one involved a fall from the sail training vessel Pelican of London's gangway at Sharpness, Gloucester and the other resulted from a fishing gear failure on board FV Honeybourne III off Beachy Head, East Sussex. Both cases are now on our current investigations list.

An investigation has been added to our current investigations list following the capsize of fishing vessel Lexi Rose (BF370) off Macduff, Aberdeenshire, Scotland with loss of 1 life on 21 September 2023.

A propulsion failure on the UK registered passenger vessel Ocean Diva (9938078), which then made contact with a moored barge at Crossness, Halfway Reach on the River Thames, London on 22 June, has been added to our current investigations table.

The main engine shutdown of the Malta registered ro-ro cargo ferry Mazarine and the subsequent breach of its underwater hull on Wolf Rock off Land’s End, Cornwall, England, has been added to the list of investigations being led by other administrations. Malta is investigating, with the UK declared as a substantially interested State.

Our investigation table has been updated to remove the Scot Carrier/Karin Høj case following publication of our investigation report and to add an investigation into the total constructive loss of the commercial swim event support vessel, Channel Queen.

Today, the MAIB has published its interim report on the investigation of the capsize and foundering of the UK registered stern trawler Njord (SH 90) in the North Sea, approximately 100 miles west of Stavanger in Norway, resulting in the death of 1 crew member on 6 March 2022.

Our investigations table has been updated to add the grounding of the 11m Anguilla registered commercial day excursion boat Calypso 2 on the north-west coast of Anguilla on 11 March 2023. The 2 crew and 4 passengers were thrown overboard resulting in several injuries and the loss of 2 lives. MAIB is investigating on behalf of the Governor of Anguilla. The status for two cases (Scot Carrier/Karin Høj and RRS Sir David Attenborough) have been updated to consultation stage.

A new case has been added to the current investigations table following the machinery failure and subsequent grounding of the UK registered ro-ro passenger ferry Pentalina at St Margaret's Hope, South Ronaldsay, Scotland, on 29 April 2023. The status for the investigation into the UK's emergency response to a fatal sinking of an unnamed migrant boat on 24 November 2021, has also been updated to 'Consultation stage'.

A case has been added to the current investigations table after a fatal fall of an engine room crew member down a ventilation duct on board the Cayman Islands registered bulk carrier Equinox Seas (IMO: 9229697), at a shipyard in Ermoupoli, Syros, Greece on 17 April 2023. This Preliminary Assessment is being conducted on behalf the Government of the Cayman Islands/Maritime Authority of the Cayman Islands, in accordance with the Memorandum of Understanding between the MAIB and the Red Ensign Group Category 1 registries.

The 'investigations into accidents involving UK registered vessels and/or UK citizens that, by agreement, are being led by other administrations' table has been updated to advise that Liberia are investigating a missing crew member believed to have fallen overboard from the Liberian registered drill rig jack up vessel Valaris 121, approximately 98nm off Aberdeen, Scotland, while under tow on 22 January 2023. Please note the UK’s Health and Safety Executive (HSE) is also conducting an investigation.

The status of the Harriet J investigation has also been amended to 'Consultation stage'.

The investigations being led by other administrations section on our current investigations page has been updated following an engine room fire and loss of power on the Cyprus registered ro-ro passenger ferry Isle of Innisfree. The ferry was on passage from Dover, England to Calais, France on 3 March 2023. BEAmer (France) is investigating, with the UK and Cyprus declared as substantially interested states.

A new case has been added to the current investigations table following a fatal fall from height on boarding of the Finland registered ro-ro cargo ship Finnhawk from the UK registered pilot vessel Humber Saturn, off the Humber Estuary, England on 8 January 2023.

A preliminary assessment is ongoing into the sinking of a migrant boat during an attempt to cross the English Channel on 14 December 2022, resulting in 4 confirmed fatalities. The exact number of persons in the boat has not yet been established. This case has been added to MAIB's current investigations table.

We are investigating the capsize of the 7.47m UK registered fishing vessel Crig-A-Tana, resulting in its loss, 3.7nm south-east of Bass Point, Cornwall, England. The two crew abandoned to a liferaft and were rescued uninjured.

We have issued an interim report on the investigation of the search and rescue response to the accident involving a small migrant vessel, resulting in the loss of at least 27 lives in the Dover Strait on 24 November 2021. A link to this report is located in the 'Status of investigation' column of our 'Current Investigations' table. We have also added a new investigation to the table on the collision of the Malta registered chemical tanker Ali Ka with a berth at Canvey Island, resulting in damage to both vessel and berth on 25 October 2022.

On behalf of the government of the British Virgin Islands, we have started a new investigation into a mechanical failure, and the consequential uncontrolled turn of a powerboat, which caused the ejection of 10 of the 11 occupants, resulting in multiple injuries and the loss of 2 lives off Totola, British Virgin Islands on 2nd October 2022.

We have started a new investigation into the grounding and loss of UK registered fishing vessel Ocean Maid (BA55) on Cairnbulg Point, Aberdeenshire on 24th October 2022. The crew abandoned to a life raft and were rescued uninjured.

We have started a preliminary investigation into the collision between the fishing trawler Kirkella and the moored tug Shovette in King George dock, Hull, on 24/6/22 resulting in the sinking of the tug.

We've started an investigation into the capsize of a motor vessel on Roadford Lake in Devon, England on 8 June 2022. The vessel was carrying six occupants, four have been rescued but two disabled persons still remain unaccounted for.

Last week the MAIB deployed a team of inspectors to conduct a preliminary assessment into a fatal accident that occurred on 12 January on board the privately-owned motorboat Emma Louise in Hamble, Hampshire, England. A decision about whether a safety investigation will be launched will be made by the Chief Inspector following a review of the circumstances of the accident and the findings of the assessment.

We are investigating last week's collision between the UK registered general cargo vessel Scot Carrier and the Danish registered construction vessel Karin Høj off the coast of southern Sweden, resulting in 2 fatalities.

We have started an investigation following multiple fatalities during a stand-up paddleboard activity on the River Cleddau, near Haverfordwest, Wales on 30 October 2021.

We have started a preliminary assessment into a fatal man overboard from the single-handed UK registered fishing vessel Harriet J (AH180) near the port of St Abbs in south-east Scotland on 28 August. An investigation into the fatal injury to a crewman during mooring deck operations on board the Isle of Man registered bulk carrier Teal Bay (9343637) in the Kavkaz anchorage, Russia on 30 August, is also underway. This investigation is on behalf of the Isle of Man Ship Registry, in accordance with the MOU between the MAIB and Cat 1 Red Ensign Group.

We have started an investigation into a fatal injury to a crewman during mooring operations on board the Isle of Man registered bulk carrier Mona Manx while berthing at Las Ventanas, Chile on 26 August. This investigation is on behalf of the Isle of Man Ship Registry, in accordance with the MOU between the MAIB and Cat 1 Red Ensign Group.

We have started two new investigations into accidents that occurred last week. At about 0330 on 25 July the Portuguese registered general cargo vessel BBC Marmara (9454228) grounded on Eilean Trodday, north of Skye, Scotland and on 29 July, there was a fatal man overboard from the UK registered fishing vessel Pioneer (NN200) off Hastings, England.

A preliminary assessment is underway into a fatal man overboard from the single-handed UK registered creel boat St Peter, near the port of Cove, south of Dunbar, south-west Scotland on May 2.

A preliminary assessment is underway into a serious injury to crew member following the failure of part of the equipment during lifting operations on board the fish farm support workboat Annie E near the Island of Muck, Inner Hebrides, Scotland on April 3.

We have started a investigation into the injuries sustained by crewmembers inside a lifeboat that rolled onto its side and then fell overboard during a launching drill on the research vessel RRS Sir David Attenborough in Loch Buie, Scotland on 5 March 2021.

We have launched an investigation into the loss of the Conwy-based fishing vessel Nicola Faith and its three crew. The investigation will look to establish the causes and circumstances leading to the loss of this vessel.

We have started Preliminary Assessment following the extensive shock damage, including hull penetration and flooding, to a potting fishing vessel, Galwad-Y-Mor (BRD116), after an external explosion off Cromer, Norfolk on 15 December.

We have started a new investigation into the capsize and sinking of UK registered scallop dredger Joanna C (BM265), south of Newhaven on 21/11/2020 with the loss of one life, and one crewman missing. Our Chief Inspector has issued a Notification of Direction to Prohibit Access to and/or interference with the wreck of the fishing vessel or its equipment and gives direction requiring that they shall be left undisturbed until further notice, pending our investigation.

We have started two new investigations: The loss overboard of 33 ISO containers from Francisca (9113214) near Duncansby Head, Scotland on 31/10/2020 and, a collision between Panamanian cargo vessel Talis (9015424) and UK registered fishing vessel Achieve (HL 257), resulting in the subsequent sinking of the fishing vessel, on 8/11/2020.

MAIB has started an investigation into the contact of the passenger vessel Waverley with the pier at Brodick, Isle of Arran, Scotland on 3 September 2020, resulting in passenger injuries and damage to the vessel.

We have started an investigation into the causes and circumstances of the fatal person overboard from the Broads cruiser Diamond Emblem on the River Bure, Great Yarmouth on 19th August 2020. Our inspectors have visited the site and the vessel involved to gather evidence, enquiries are ongoing.

We have started an investigation into the fatal injury to a passenger on the RIB Seadogz, following a collision with a navigation marker in the Solent this weekend.

We have recently started two new investigations: a fatal injury to passenger on a RIB following a collision with a jetski off Menai Bridge on 8 August and, investigating on behalf of the Isle of Man Ship Registry, a fatal injury to crewman following an engine room fire on the LPG tanker Moritz Schulte (9220794) in Antwerp on 4 August.

The investigation into the capsize and foundering of fishing vessel Investor while east of Ardnamurchan Point, Scotland on 3 February 2019 has been discontinued in accordance with The Merchant Shipping (Accident Reporting and Investigation) Regulations 2012 Para 11(9). The reason for this decision is that the issues identified by the investigation have already been the subject of MAIB safety recommendations. Nevertheless, the MAIB may, in the future, publish the story as an anonymised safety digest article.

An investigation has begun into an accident at Jebel Ali, United Arab Emirates, on 4 May 2017. The UK registered container ship CMA CGM Centaurus contacted the quay while berthing. There are no reported injuries or pollution. Damage has been sustained to the ship, the quay and two shore cranes.
