MOD form 765 requesting amendments to MAM-D should be submitted via email to the MAA Regulations CAw4 team at DSA-MAA-Reg-CAw4@mod.gov.uk.

MOD form 765 requesting amendments to MAM-P should be submitted via email to DSA-MAA-MAMP-Enquiries@mod.gov.uk.
